I may collect background information, GP/personal details, family details, brief medical history, social circumstances, mental health history, expectations of therapy and an agreed treatment plan. This information will be collected verbally and a written record will be kept in note form taken after the session. Personal data is stored and processed in line with the Data Protection Act 1998.

• Handwritten notes and personal details are kept separate in a locked cabinet

• Email addresses are stored in a secure business mailing system
